Puppy Health Concern Form Please use this form for General Health Concerns, to Request an appointment to get a Health Certificate for Traveling with your Puppy, Heartworm Preventative Refill Requests, or for any other non-emergent, health related request. If you are requesting more Heartworm Preventative, please include your puppy's current weight in your request.
