  1. Puppy Health Concern Form Please use this form for General Health Concerns and Heartworm Preventative Refill Requests. If you are requesting more Heartworm Preventative, please include your puppy's current weight in your request.
  2. Puppy Sitter Request Sheet Puppy Raisers, please submit two weeks in advance. Puppy Sitters, please reach out to the Puppy Raiser after you sign up.

Food Ordering

  1. Local Puppy Food Pick-Up Request Form   LOCAL RAISERS please submit two weeks before food runs out. FSD staff will reach out to coordinate a pick-up date/time with you. Requests on short notice may not be able to be honored.  
  2. Purina for Professionals Local & Satellite Raisers can sign up here to have food delivered directly to their home, for a discounted rate, at their own expense.
  3. Satellite Puppy Food Request Form SATELLITE RAISERS please submit two weeks before food runs out. FSD staff will place orders each week and have the food delivered to your home. Requests on short notice may not be able to be honored.
